Diagnosis and management of abnormal uterine bleeding including irregular, heavy, and postmenopausal bleeding.
Abnormal uterine bleeding (AUB) refers to any uterine bleeding that is irregular in frequency, duration, or volume. CAUSES (PALM-COEIN): • Structural: Polyps, Adenomyosis, Leiomyoma (fibroids), Malignancy & hyperplasia • Non-structural: Coagulopathy, Ovulatory dysfunction, Endometrial, Iatrogenic, Not otherwise classified EVALUATION: • Clinical assessment and menstrual history • Pelvic ultrasound • Endometrial biopsy / D&C • Hysteroscopy when indicated • Blood tests (full blood count, coagulation profile, hormones) TREATMENT: • Medical: Progesterone, combined oral contraceptives, tranexamic acid, iron supplements • Surgical: D&C, endometrial ablation, myomectomy (for fibroids), hysterectomy (definitive)
